CCP-Eligible Program Material Changes Disclosure Form

This form should be used by authorized representatives of CCP-Eligible Programs to notify the ICVCM of any potential or actual material changes to their program or the manner in which the program is structured or operated. Material change notifications should be made as far in advance as possible.
